Imagine Pediatrics is seeking a Behavioral Health Care Manager to provide case management services to medically complex children and their families. The role involves monitoring patients, conducting assessments, creating care plans, providing interventions, and collaborating with various healthcare professionals and external resources. This position requires a commitment to improving the lives of children with special health care needs.

Responsibilities

• Monitor high-risk pediatric patients
• Conduct biopsychosocial assessments
• Create and evaluate care plans
• Provide intervention for mental health crises
• Facilitate case management
• Resource external services
• Provide psychoeducation
• Collaborate with external care teams
• Participate in interdisciplinary team consultations
• Represent Imagine Pediatrics

Requirements

• Masters’ degree in social work or related field
• Provisional licenses (LMSW, PLPC, LAMFT) preferred
• 3-5 years of post-graduate experience in behavioral health case management
• Experience with pediatric population and family systems
• Proficiency in motivational interviewing
• Knowledge of social work and crisis intervention
• Experience with telehealth
• MS Office Suite proficiency
• Bilingual Spanish required
• Knowledge of behavioral health principles
• Proficient in trauma-informed care
• 10% travel for training
