Imagine Pediatrics is seeking a Neurodevelopmental Care Manager to provide compassionate care to families and children with neurodevelopmental needs. This role involves conducting assessments, formulating care plans, providing case management, and advocating for clients within various support systems. The position requires a strong understanding of neurodevelopmental diagnoses and a commitment to enhancing the quality of life for children with medical complexity.

Responsibilities

• Conduct biopsychosocial assessments
• Formulate care plans
• Evaluate care plan effectiveness
• Provide ongoing case management
• Offer family support and education
• Advocate for clients in IEP/504 planning
• Coordinate resource referrals
• Collaborate with care team professionals
• Respond to crisis situations
• Maintain accurate documentation

Requirements

• Master’s degree in social work or related field (MSW or LMSW)
• 3-5 years of post-graduate experience in healthcare social work/case management
• Experience with pediatric populations and family systems
• Knowledge of neurodevelopmental diagnoses
• Knowledge of IDEA, ADA, Medicaid, and social services systems
• Bilingual Spanish
• 10% travel for training
• Ability to work afternoons and evenings
