Imagine Pediatrics is seeking a Care Team Assistant to provide clinical and clerical support in a virtual Value-Based Care environment, focusing on children with special health care needs. The role involves phone triage, lab result delivery, pre-visit preparation, appointment reminders, and assisting with reports and referrals.

Responsibilities

• Answer and return patient calls, perform triage, and document in EHR
• Deliver lab results to patients
• Prepare for patient visits by gathering records
• Remind patients of appointments and onboard new patients
• Complete forms for provider signature
• Assist with reports and identify vulnerable patients for referrals

Requirements

• High-school level education or equivalency
• Medical Assistant Certification or 2+ years clinical experience
• Spanish fluency preferred
• CST timezone required
