Job Description

Imagine Pediatrics is seeking a Pediatric Home Care Specialist (Paramedic Certified) to facilitate in-home delivery of episodic and longitudinal care to children with special health care needs. The role involves collaborating with an interdisciplinary team, providing high-quality care, assisting with urgent needs, and engaging with new and recently discharged members.

Responsibilities

• Travel to members’ homes for acute and episodic care
• Provide high-quality, compassionate care to pediatric members
• Assist with urgent needs under physician guidance
• Assist with clinical intake visits for new members
• Engage with recently discharged members to prevent readmission

Requirements

• EMT-Paramedic license in Louisiana or NREMT
• PALS, ACLS, and BLS certifications
• 3+ years of field clinical experience
• Driver’s license and reliable transportation
• Strong critical thinking and communication skills
• Experience with technology platforms
• Bilingual Spanish preferred
