Job Description

Imagine Pediatrics is seeking a Pediatric Home Care Specialist (Paramedic Certified) to deliver in-home care to children with special health care needs. The role involves collaborating with an interdisciplinary team, providing acute and episodic care, assisting with urgent needs, and engaging with families to prevent readmissions.

Responsibilities

• Travel to members’ homes for acute and episodic care
• Provide high-quality, compassionate care to pediatric members and families
• Assist with urgent needs under physician guidance
• Assist with clinical intake visits for new members
• Engage with recently discharged patients to prevent readmission

Requirements

• EMT-Paramedic license in Texas and/or NREMT
• PALS, ACLS, and BLS certifications
• 3+ years of field clinical experience
• Driver’s license and reliable transportation
• Strong critical thinking and communication skills
• Experience with technology platforms
