Job Description

Imagine Pediatrics is seeking a Patient Engagement Associate II to enroll and onboard children and caregivers into their program. This role involves research, telephonic outreach to patients and healthcare professionals, handling inbound calls, and collaborating with specialists to support enrollment. The ideal candidate will have strong communication, problem-solving, and relationship-building skills, with a passion for improving healthcare for children with special needs.

Responsibilities

• Conduct patient research to understand healthcare needs
• Perform telephonic outreach to caregivers and healthcare professionals
• Handle inbound calls from caregivers and coordinators
• Develop knowledge of SSI program and perform outreach
• Collaborate with specialists for embedded outreach
• Support re-engagement of enrolled patients

Requirements

• 2+ years of healthcare experience in care coordination, community health, or patient engagement
• Proven success in a contact center environment
• Strong documentation and CRM experience preferred
• Excellent communication and active listening skills
• Ability to build trust and relationships
• Strong research and problem-solving skills
• Reliable transportation and internet access
• Bilingual Spanish required (in Florida market)
