Job Description

Imagine Pediatrics is seeking a Patient Engagement Associate II to enroll and onboard children and caregivers into their program. This role requires strong critical thinking, problem-solving skills, and collaboration to understand patient needs, perform outreach, and ensure a seamless onboarding experience.

Responsibilities

• Conduct patient research to understand healthcare needs
• Perform telephonic outreach to caregivers and healthcare professionals
• Handle inbound calls from caregivers and health plan coordinators
• Develop knowledge of SSI program and perform outreach
• Collaborate with Provider Engagement Specialists for embedded outreach

Requirements

• 2+ years of healthcare experience in care coordination, community health, or patient engagement
• Proven success in a contact center environment
• Strong documentation skills and CRM experience preferred
• Excellent communication and active listening skills
• Ability to build trust and relationships
• Strong research and problem-solving skills
• Reliable transportation and internet access
• Bilingual Spanish required
