Job Description

Imagine Pediatrics is seeking a Licensed Practical Nurse to provide support to patients post-hospital discharge, focusing on education, care coordination, and ensuring access to necessary resources. This role involves telephonic and virtual visits, collaboration with healthcare teams, and proactive engagement with patients to avoid future emergency department visits.

Responsibilities

• Engage patients post-ED visits and provide education
• Conduct virtual/telephonic visits for care coordination
• Identify symptom worsening and escalate to clinical team
• Provide education on Imagine Pediatric Services
• Ensure follow-up appointments are scheduled
• Coordinate with discharge planners and Transition of Care Nurses
• Attend daily team huddles
• Liaison with hospitals and health systems
• Provide referrals to internal/community resources

Requirements

• Associates degree or higher preferred
• 2+ years of clinical work experience
• Emergency or inpatient experience preferred
• Experience with medically complex patients preferred
• Pediatrics experience preferred
• Licensed Practical Nurse required
• Knowledge of Medicaid population preferred
• Bilingual Spanish required
