Job Description

Imagine Pediatrics is seeking a Licensed Vocational Nurse to provide support to patients post-hospital discharge, focusing on education, care coordination, and ensuring access to necessary resources. This role involves telephonic and virtual visits, collaboration with healthcare teams, and proactive engagement with patients to avoid future emergency department visits.

Responsibilities

• Engage patients post-ED visits and provide education
• Conduct virtual and telephonic visits for care coordination
• Escalate symptom worsening to clinical team
• Provide education on Imagine Pediatric Services
• Ensure follow-up appointments are scheduled
• Coordinate with discharge planners and Transition of Care Nurses
• Attend daily team huddles
• Liaison with hospitals and health systems
• Provide referrals to internal and community resources

Requirements

• Associates' degree or higher preferred
• 2+ years of clinical work experience
• Licensed Practical Nurse required
• Experience with medically complex patients preferred
• Pediatrics experience preferred
• Knowledge of Medicaid population preferred
• Bilingual Spanish required
