Job Description

Pomelo is seeking a Care Coordinator to support the clinical practice's efficiency by managing operational and administrative processes. This role involves care navigation, patient support, eligibility checks, scheduling, referral management, and workflow development to improve care coordinator efficiency.

Responsibilities

  • Provide care navigation support to patients
  • Monitor member support phone line and answer patient questions
  • Complete eligibility checks for patients
  • Manage patient scheduling and provider schedules
  • Manage referrals to health plan’s Case Management team
  • Develop new workflows to improve efficiency

Requirements

  • 4+ years healthcare administration experience
  • Experience with virtual care preferred
  • Familiarity with SDOH platforms and referrals
  • Results-oriented and meticulous
  • Comfort with ambiguity
  • Ability to work cross-functionally
  • Passion for maternal and child health
