Job Description

Pair Team is seeking a motivated full-time Lead Care Manager to engage with individuals experiencing Serious Mental Illness/Substance Use Disorder, homelessness, and/or high medical needs. The role involves outreach, engagement, and support in accessing whole-person care, with a focus on building trust and relationships. The position is primarily remote with occasional on-site community visits.

Responsibilities

• Maintain caseload using evidence-based approaches
• Support members with social support navigation
• Conduct outreach via phone and SMS
• Identify and incorporate health goals into care plans
• Support care team with delegated tasks
• Collaborate on care issues through case reviews
• Meet monthly encounter metrics
• Break down barriers to program continuation
• Assist with community support connections
• Coordinate care management appointments
• Utilize online platforms for collaboration

Requirements

• 1+ years of general work experience (Case Management preferred)
• Located in Orange County, CA
• Bilingual – English/Spanish
• Strong understanding of cultural fluency
• High degree of empathy
• Collaborative team player
• Organizational skills
• Strong technical skills (CRM, Excel, Word)
• Valid driver’s license and auto insurance
