Job Description

Pair Team is seeking a highly motivated, full-time Behavioral Health Care Manager to provide mental health resources and therapeutic interventions for individuals with serious mental illness, substance use disorder, homelessness, or high medical needs. The role involves working in a team-based model with a focus on improving quality of life through evidence-based behavioral interventions, with primarily remote work and occasional on-site visits.

Responsibilities

• Provide short-term interventions and behavioral activation strategies
• Provide intermediary interventions as a bridge to community supports
• Provide brief crisis intervention and warm hand-offs
• Assist with linkage to ongoing behavioral health treatment
• Support team caseload to address barriers to care
• Educate individuals on behavioral health conditions
• Identify health/wellness goals and incorporate into care plans
• Collaborate on care issues through case reviews
• Track and manage caseloads using electronic medical records
• Consult with team about clinical concerns

Requirements

• LCSW, LMFT, or LPCC license in California
• Bilingual English/Spanish
• Strong understanding of cultural fluency
• Experience working with individuals experiencing complex chronic needs, homelessness, or Severe Mental Illness/Substance Use Disorder
• Strong technical skills and comfort with technology
• Physical location in or near San Diego, California
• Reliable transportation
