Job Description

Pair Team is seeking a highly motivated, full-time Behavioral Health Care Manager to provide mental health resources and therapeutic interventions for individuals with serious mental illness, substance use disorder, homelessness, or high medical needs. The role involves working in a team-based model, primarily remotely with occasional on-site visits, to improve patients' quality of life through evidence-based behavioral interventions.

Responsibilities

• Provide short-term interventions and behavioral activation strategies
• Provide intermediary interventions and referrals to community supports
• Provide brief crisis intervention and warm hand-offs
• Link individuals to ongoing behavioral health treatment
• Address barriers to care
• Educate individuals on behavioral health conditions
• Identify health/wellness goals and incorporate them into care plans
• Collaborate with the Enhanced Care Management team
• Track and manage caseloads using electronic medical records
• Consult on clinical concerns and provide educational training

Requirements

• LCSW, LMFT, or LPCC license issued by the state of California
• Bilingual English/Spanish
• Strong understanding of cultural fluency
• Experience working with individuals experiencing complex chronic needs, homelessness, or Severe Mental Illness/Substance Use Disorder
• Strong technical skills and comfort with technology
• Physical location in or near San Diego, California
• Reliable transportation
