Job Description

Pair Team is seeking a motivated Registered Nurse Care Manager to support patient-driven care plans and improve outcomes for individuals with complex medical needs, including those with Serious Mental Illness, Substance Use Disorder, experiencing homelessness, or with high medical needs. The role involves remote work with occasional on-site community visits, focusing on health education, care coordination, and collaboration within an interdisciplinary team.

Responsibilities

• Support individuals with complex medical needs
• Identify health/wellness goals and incorporate into care plans
• Educate on medical and behavioral health conditions
• Provide medication reconciliation
• Coordinate prescriptions and prior authorizations
• Track assessments and screenings
• Collaborate with multidisciplinary care team
• Triage escalations and provide brief interventions
• Participate in systematic case reviews
• Consult on clinical concerns and provide training
• Build trust with individuals experiencing homelessness or with mental illness
• Engage individuals in care using relationship-based strategies

Requirements

• Active Registered Nurse license in California
• Located in California
• 2-3+ years of experience in care coordination or case management
• Experience working for a health plan or at-risk provider
• Bilingual – English/Spanish
• Strong technical skills and comfort with technology
• HIPAA-compliant at-home work environment
• Understanding of cultural fluency
• Experience working with individuals with complex needs
• Empathetic and driven to reduce healthcare barriers
