Form Health is a virtual obesity medicine clinic seeking experienced Nurse Practitioners to provide telehealth care for patients with obesity. The role involves collaborating with physicians and registered dietitians to deliver team-based care, focusing on evidence-based lifestyle counseling and pharmacotherapy.

Responsibilities

• Provide longitudinal telehealth care for patients with obesity
• Collaborate with physicians and registered dietitians
• Deliver team-based care
• Conduct telemedicine visits
• Provide evidence-based lifestyle counseling and pharmacotherapy

Requirements

• Nurse Practitioner degree with obesity medicine experience
• Active license to practice and prescribe in Washington
• 2+ years of experience in outpatient, obesity medicine, endocrinology, or bariatrics
• Excellent interpersonal skills
• Commitment to high-quality, evidence-based medical care
• Prior telemedicine experience
• Spanish fluency preferred
