Job Description

Form Health is seeking a dedicated and collaborative Obesity Medicine Physician to join their multidisciplinary weight management team. The role focuses on providing direct patient care, supervising Advanced Practice Providers (APPs), and ensuring high-quality, evidence-based obesity treatment through telemedicine.

Responsibilities

• Provide direct patient care focusing on obesity treatment
• Review charts and conduct return visits for APP patients
• Provide treatment planning and medical management support
• Prescribe obesity medications
• Conduct collaborative meetings with APPs
• Ensure consistent clinical documentation and protocols
• Assist in the professional development of APPs
• Participate in team-based care initiatives
• Attend departmental meetings and contribute to quality improvement

Requirements

• MD or DO degree and ABOM certification
• Minimum of 5 years as an attending physician
• MA medical license and insurance credentialing
• Excellent interpersonal and communication skills
• Strong interest in mentoring and collaborating with APPs
• Experience working with APPs or trainees
• Adherence to evidence-based obesity treatments
• Commitment to patient-centered care
• Comfortable working in a multidisciplinary team
