Job Description

Form Health is seeking a Dedicated and Collaborative Obesity Medicine Physician to join their multidisciplinary weight management team. This role focuses on providing direct clinical services and structured supervision for Advanced Practice Providers (APPs) in delivering high-quality patient care for obesity treatment.

Responsibilities

• Provide direct patient care focusing on evidence-based obesity treatment
• Review charts and conduct return visits for APP patients
• Provide treatment planning and medical management support
• Prescribe non-controlled obesity medications
• Conduct weekly collaborative meetings with APPs
• Ensure consistent clinical documentation and protocols
• Assist in the professional development of APPs
• Participate in team-based care initiatives
• Attend departmental meetings and contribute to quality improvement

Requirements

• MD or DO degree and ABOM certification
• Minimum of 5 years as an attending physician
• Licensure to practice and prescribe in NY
• Excellent interpersonal and communication skills
• Strong interest in mentoring and collaborating with APPs
• Experience working with APPs or trainees
• Adherence to evidence-based obesity treatments
• Commitment to patient-centered care
• Comfortable working in a multidisciplinary team
