Form Health is seeking a Registered Dietitian to provide individualized medical nutrition therapy to patients through a telehealth platform, focusing on obesity medicine and weight management. The role involves collaborating with physicians and offering comprehensive support via video visits and daily messaging.

Responsibilities

• Provide medical weight management support via video visits
• Establish professional relationships with patients
• Create customized nutrition plans
• Provide nutrition education and counseling
• Review patient food photos
• Support patients via text messaging
• Lead patient support groups
• Enhance the medical weight loss program

Requirements

• Registered Dietitian with 3-5 years of experience
• Experience in counseling and behavior modification
• Experience working with patients with obesity
• CSOWM certification preferred
• Experience in medical weight loss clinics
• Experience with weight loss medication and meal replacement plans
• Strong cognitive-behavioral therapy counseling techniques
